If you are new to the Middleberg District and have not enrolled your child in 2024-25 academic year, please complete an enrollment form and send it to lwittenbach@middleberg.k12.ok.us as soon as possible. Other required documentation is birth certificate copy, immunization record and proof of Middleberg residence. Enrollment forms and documentation can also be dropped off in the school office Mon-Thurs 8am to 1pm. 2130 County Road 1317, Blanchard, OK. Pre-k enrollment is on a first come, first served basis.
