We are now enrolling pre-k students for the 2025-26 academic year. Pre-k students must be 4 years old by September 1st, 2025 to enroll. Pre-k is filled on a first come, first serve basis. In order to enroll parents must provide the following documents:
Completed Middleberg Enrollment Forms found here
Copy of Official Birth Certificate
Copy of Most Recent Immunization Records
Proof of Middleberg District Residence such as a utility bill
Parents can drop off enrollment forms and documents to our elementary office during school hours or email forms to thensley@middleberg.k12.ok.us
